BILLING & INSURANCE

We participate in most major private insurance plans and Medicare. Co-payments are expected at the time of service. Please understand that you will also be responsible for any deductibles, coinsurance, and/or other costs not covered by your insurance plan. If a referral is required for your visit, it is your responsibility to obtain the referral before your appointment. We offer discounts for self-pay patients who pay in full at the time of service.

We bill insurance as a courtesy to our patients. Patients are responsible to know their insurance coverage as well as responsible for the bill.

Note insurance plans change regularly and patients are advised to call their insurance company for confirmation. If your insurance company requires that you need a referral you are responsible to provide one from you primary care doctor.​
