OUR POLICIES

LET'S KEEP EVERYONE SAFE

Patients must still arrive with a mask and keep the mask on during office visits, even if you have been vaccinated.   If you have questions about our mask policy, please contact us.

ALL PATIENTS MUST COMPLETED PRIOR TO SEEING THEIR PROVIDER

All patients must complete a patient information form before seeing their provider; these forms will be available online through the website. If it your first appointment please arrive 30 minutes early as you may be asked to complete some additional forms. If you are an established patient please arrive at least 20 minutes before the appointment to provide new insurance information and update registration information.

CANCELLATIONS | NO SHOWS

PLEASE READ OUR CANCELLATION POLICY

Your appointment will be confirmed via our automated system 3 days prior to your visit. If you are unable to keep your appointment, we request 48-hour notice for cancellations. We realize that sometimes emergencies arise, and patients must miss, reschedule, or cancel their appointments on short notice. However, missed appointments affect our ability to provide timely appointments for all patients. We reserve the right to dismiss patients from our practice after three missed appointments in a 12-month period. A $50 no show fee will be charged for all established patients who no show, reschedule or cancel less than 48-hour notice and $75 fee for all new patients who no show, reschedule or cancel with less than 48-hour notice.

MEDICATION REFILLS

PLEASE HANDLE THIS DURING YOUR OFFICE VISIT

Medication refills should be handled at the time of your office visit. If you need medication refills outside of your visit, please contact your pharmacy or send a request through the patient portal. All prescription refill requests should be called into your pharmacy or requested through the portal at least 5 - 10 business days before the last medication is taken to allow adequate time for approval. Refills will only be handled during normal business hours, Monday through Thursday. The cut-off time to request refills is 3:00 pm. All refill requests that are requested on Fridays will be processed the following business day.

FMLA | DISABILITY PAPERWORK

FORM FEE OF $25

Forms will be completed if approved by your provider. Our form fee is $25.00 for each set of forms that require the physician signature. This must be paid in full before the paperwork can be picked up or faxed. Please allow 7-10 working days to complete paperwork. Medical forms CANNOT be completed on the same day forms are presented to the office.

MEDICAL RECORDS

ALLOW 7-10 BUSINESS DAYS TO BE PROCESSED

All Medical Records are processed by an outside company (Vital Records). If you need medical records, then please request in writing and allow 7 -10 business days for this to be processed.
